We begin creating a care plan by setting objectives and goals and determining what actions need to be taken in order to achieve those goals—and in what time frame. For example:

  • Medical and non-medical resources and services are identified.
  • Multiple medical appointments and services are scheduled, coordinated, and monitored.
  • All parties, local and out-of-state, are communicated with regarding observation, direction, research and recommendations.

Throughout the entire care coordination process—no matter how long it takes, no matter how long services are needed—Care for You is committed to supporting you, your family and your loved ones.

We ensure that:

  • Continuing issues and concerns are addressed.
  • You receive proper, consistent, and unwavering advocacy.
  • Legal documents—that you or a loved one might use to transfer financial or medical decisions to another person—are made available, understood, and put in the hands of the right people.

Whenever possible these services are performed in person. If the need arises, phone, email, and written reports may also be utilized.
